Home care provides clinical and supportive services for individuals recovering from illness, injury, or managing chronic health conditions, all from the comfort of home. Home care services are ordered by a physician, require skilled care from a trusted professional (e.g. a nurse or physical therapist), and are paid for by Medicare or other health insurances.

Croí Health Offers
Skilled nursing and in-home nursing care manages the plan of care and provides an assessment of the patient’s condition and needs. Developing a mutually agreed-upon, person-centric care plan, the clinical team offers the patient, family and/or caregivers education and information to maintain the patient’s health, safety, and independence. Nurses respond to a variety of clinical needs, including:
- Medication management
- Post-surgical care
- Disease management and care
- Dietary consultation and education to support the patient’s self-management of their disease and/or condition
Palliative care is an option available at any point during a serious illness and does not depend on one’s prognosis. It is a holistic approach to relieving suffering and providing quality of life so patients can live as fully as possible. Patients can still pursue treatments while under palliative care. Those living with serious illnesses find that palliative care helps with symptoms like shortness of breath, fatigue, pain, loss of appetite and nausea.
Social workers assess the patient’s physical, emotional, environmental, and social needs and then provide the patient with supportive resources in response to identified concerns and issues.
Patient advocacy is also offered, as necessary. Croí Health employs master's-level licensed social workers.
Encompassing physical, occupational, and speech therapy, our rehab department employs licensed professionals who are certified in their respective areas of expertise. Therapists work individually or as a team depending on the patient’s individual needs. Home assessments are often offered by the physical therapist and/or occupational therapist with recommendations and suggestions for providing a safe, risk-free environment, as well as enhancing the patient’s functioning level.
Aides provide personal care and assistance with activities of daily living such as bathing, dressing, toileting, and meal preparation. All Croí Health home health aides are certified according to the Massachusetts Department of Health regulations and trained specifically to Croí Health's standards of care. All care by our home health aides is provided through the direction of a clinician, such as a nurse or physical therapist.
Private pay is an adjunct service to complement existing certified home health care or hospice care, or a stand-alone to meet needs for companion care or homemaking. These typically are not covered by insurance, although some long-term care policies and/or veteran benefits may help subsidize costs.
