Hospice is not a place but rather a concept of care that offers physical, emotional, social, and spiritual support for patients and families. Our hospice care provides professional medical care with emphasis on pain management and symptom relief. Hospice care can also be provided at our Pat Roche Hospice Home or in a facility.
We respect and honor the right of every patient and family to participate in the final stage of life’s journey and encourage our patients and families to participate in the design of their plan of care.

What is Hospice Care?
Hospice provides comfort-focused care for individuals with a terminal illness, emphasizing quality of life by focusing on what is important for each patient. Patients, families, caregivers, and hospice professionals participate in care designed to best meet individual patient needs.
Our team works alongside each patient’s physician to coordinate the best approach for pain and symptom management. Home-based visits are complemented by 24/7 access to our on-call hospice line.
We work directly with the medical community, families, and caregivers to ensure that every hospice patient is treated with dignity and respect. Mindful of our patients’ needs, our chaplains provide a uniquely dignified spiritual presence with heart for all our patients.
When is Hospice Appropriate?
Hospice is for many diagnoses (e.g. Alzheimer’s, cancer, COPD, heart-disease) and is elected when, according to Medicare eligibility, there is an advanced life-limiting illness and any of the following:
- Pain or other forms of suffering
- A choice to focus on comfort care and quality of life
- The family needs trained help and support to manage end-of-life care in conjunction with their physician’s care

How Croí Health Provides Care
Hospice Team
When you trust Croí Health for your hospice services, patients will have a team behind them to ensure comfort and support. In addition to a nurse case manager, medical social worker, and home health aides, the family can also request physical and occupational therapists, speech therapists, spiritual care coordinators, and volunteers.
Emotional
The entire Croí Health team doesn’t just assist the patient but provides support to the family as well. With extensive counseling, grief support, and assistance with community resources, our social workers will meet with the patient and family as needed. Our end-of-life doulas work closely with the hospice team to ensure comprehensive support, focusing on offering a peaceful and meaningful experience during the final stages of life
Medical Care
Both our hospice medical director and hospice nurses are certified in hospice care and have extensive training in pain and symptom management. They will work together to provide an individualized comfort care plan based off the patient’s needs.
Spiritual
As an optional service, Croí Health chaplains are available for visits no matter what a patient’s personal beliefs and spiritual needs are. If a patient would prefer to be connected with local clergy or resources instead, our team is more than willing to make that connection.
Personal Care
Specially trained in hospice care, our home health aides provide assistance with personal needs, including bathing, dressing, skin care, and restroom assistance.
Bereavement
Hospice bereavement services are an important support system before, during, and after a loved one’s death. We provide support to surviving family members for up to 13 months following the loss of a loved one.

Hospice care is for people in the final months of life who are no longer seeking curative treatment, while palliative care can be provided at any stage of serious illness alongside treatments to relieve symptoms and improve quality of life.
Most hospice care is covered by Medicare, Medicaid, and private insurance plans, with little to no cost for patients. However, please check with your provider to confirm eligibility.
It is most common for patients with a life expectancy of six months or less, when focus shifts from curing the illness to quality end of life.
Yes. Hospice care can be provided in the comfort of home, within a long-term care facility, or at the Pat Roche Hospice Home in Hingham.
Hospice also supports families, offering guidance and comfort during the final weeks or months of life, as well as ongoing bereavement support after a loved one’s passing.
A patient can leave hospice care and resume other treatments. They can also re-enroll in hospice later if their condition worsens and they meet eligibility again.
